CA State Form LIC 602A - Physician’s Report for Residential Care Facilities for the Elderly (Assisted Living)

By law, people moving into assisted living residential facilities in California must have a Physicians Report for Residential Care Facilities for the Elderly, Form LIC 602A, on file upon move-in, and it must be renewed regularly per California regulations. We offer rapid, convenient, mobile onsite form 602A completion service. This service is ideal for patients in a hurry, who are unable to obtain timely Form LIC 602A completion by their primary care provider. For late morning appointments, we can have your form completed by the end of the day. Our physician, upon arrival, will perform the evaluation as required. The evaluation includes a brief medical examination and documentation of medications, and diet and activity needs and/or limitations. The doctor will then present you with two options for tuberculosis screening, which is required for the form LIC 602A evaluation.
A skin test for TB may be done by injecting a small quantity of purified protein derivative or PPD, usually on the arm. Then 48-72 hours later the injection site is checked for any inflammatory reaction. If the test is positive, a chest x-ray must be done to screen for active pulmonary tuberculosis infection. Our doctor may provide this testing. I some cases the destination residential facility can provide this test in advance in order to speed up the move-in process.
An IGRA blood test may be done to screen for tuberculosis, as well. Results are available from most labs in 1-3 days. Patients may go to their nearest preferred lab collection center, or our physician or a mobile phlebotomist may draw the blood onsite.
For patients needing the RCFE Physicians Report form completed fast, a chest x-ray can be done in place of the skin or blood test. The chest x-ray machine and technician can come to the patient or the x-ray can be done at your nearby preferred imaging facility. Chest x-ray results are usually available within 1-3 hours after being done via portable machine, and the actual x-ray can usually be done same day upon request in urban areas. We use a third party mobile xray service for patients located in the SF Bay Area, and in most cases Medicare will cover this. Outside of the Bay Area, mobile xray services are available in many, but not all, areas.
Once the TB screening results are available to our physician, the signed completed RCFE form is faxed or emailed securely to your requesting residential care facility, and a copy may be provided to you.
California Regulation 87458 states that a residential care facility must “obtain and keep on file, documentation of a medical assessment, signed by a physician, made within the last year”, and must “obtain an updated medical assessment when required by the department.”

We can complete these forms too, usually with same day turnaround depending on tuberculosis test result availability (see above). However most community care facilities have a physician available to do this, so we recommend that you ask them first.
